Full curriculum vitae
Health insurance policy with minimum coverage of 40,000 THB/year for outpatients and 400,000 THB/year for inpatients, and covering the entire duration of the applicant's stay in Thailand
Health insurance certificate signed by the insurance company confirming that the applicant is insured in accordance with the requirements for a non-immigrant "O-A" visa
Original copy of the applicant's bank statement (stamped and certified) for the past 6 months showing a balance of at least 800,000 Baht (or the equivalent) OR a certificate of income confirming a monthly income of not less than 65,000 Baht
Pension statement (if the applicant is a pension earner)
Criminal record issued by relevant authorities of the country of the applicant's nationality or residence (issued no more than three months and notarized by notary organs or diplomatic/consular mission of the applicant’s nationality)
Medical certificate issued and certified by a doctor of the country where the application is submitted, confirming that the applicant is free from the prohibited illnesses indicated in the Ministerial Regulation No. 14 B.E. 2535 (issued no more than three months and notarized by notary organs or diplomatic/consular mission of the applicant’s nationality)
Copy of air ticket to Thailand (the name of the applicant must be clearly indicated)
In case the applicant wishes to bring along a spouse who is not qualified for a non-immigrant “O-A” visa, a marriage certificate (notarized by notary organs or diplomatic/consular mission of the applicant’s nationality) must be presented, in which case the spouse's application will be considered under the criteria for a
non-immigrant “O” visa (temporary stay).
